the big data centers do have on-site redundant power,
in the case of amazon, the backups had backups.

the failure path length was quite remarkable
and the length the obvious reason it had not
been fully exercised during tests.

as for a firing offense, there's more than one 
in that picture. there are several people involved
who are supposed to catch such things and it
appears none of them did.
