A friend and colleague who sends out for PCBs a couple of times a month says http://www.myropcb.com/

Note that the setup charges of $25 are not repeated for subsequent orders of batches of the same thing unless there is a change.

They claim 10 day turnaround but he says it tends to be less.  If you are in a hurry and want it in 5 days instead of 10, be prepared to pony up an extra $16.10

You can probably see why I don't find the ferric chloride or tabletop milling machine compelling.
