I've used OSH Park in the past, and have been pleased with the results.  They use a US-based fabricator and the boards a finished with pretty high quality.  Since I've last used them, they've upgraded their web site and process for submitting your gerber files, etc.

I've only used them for low-quantity prototypes (3 boards), and haven't really investigated the pricing for higher volume production.  While not as cheap as some of the offshore alternatives, the quality seems higher and turn-around a bit faster.
